How do I replace my Birth Certificate in New Mexico?

Here’s the quick and easy way to obtain your birth certificate. Do you live in Albuquerque, Bernalillo, or any other part of New Mexico? Did you lose your birth certificate? Not to worry. We will explain what steps you’ll need to take in order to obtain an in-state or out-of-state birth certificate, where you can obtain them, and how to order them online.

IN-STATE (NEW MEXICO) BIRTH CERTIFICATE:

In order to obtain an in-state birth certificate, contact the Department of Health, New Mexico Vital Records website. You can also visit the Midtown Public Health Office. It’s located at 2400 Wellesley Avenue NE, Albuquerque, New Mexico, 87107. They’re open Monday through Friday from 9:00 AM to 3:00 PM. You must provide your proof of identity to obtain the birth certificate. An example would be a valid government ID. The approximate cost per copy is $10.00. Expedited services are available at an additional cost. 

Navajo Nation Office of Vital Records. For a Navajo Affidavit of Birth, contact the Navajo Office of Vital Records at 928-871-6383.

OUT OF STATE BIRTH CERTIFICATE: 

To obtain an out-of-state birth certificate, you must contact vital records from the state you were born in. You can go online to the National Center of Health Statistics and Vital Records website and order your birth certificate online at: https://www.cdc.gov/nchs/index.htm. If the state you were born in does not offer online ordering, this website will provide further information to obtain your out-of-state birth certificate.
